Melt Mobile Infrared Sauna Consent and Assumption of Risk
Permission to use the full spectrum infrared sauna and/or cold plunge pods is contingent on guests providing accurate answers to the questions below so that we can mutually agree on the health and well-being of the sauna user, as well as signing this agreement.
In consideration of the services of Melt Mobile Infrared LLC and their sauna provided by Sunlighten , its agents, owners, officers, affiliates, employees, and all other persons or entities acting in any capacity on its behalf (hereinafter collectively referred to as “Melt Mobile Infrared Sauna” OR "Melt Mobile sauna"), I hereby agree to release, indemnify, and discharge Melt Mobile Infrared Sauna, on behalf of myself, my spouse, my children, my parents, my heirs, assigns, personal representative and estate as follows:

1. I acknowledge that my use of an infrared sauna entails known and unanticipated risks that could result in physical or emotional injury, death, or damage to myself. The risks include, among other things, those items identified in the Precautions form attached hereto.

2. I expressly agree and promise to accept and assume all of the risks existing in this activity and the use of Melt Mobile Infrared Sauna facilities and equipment. My participation in this activity is purely voluntary, and I elect to participate in spite of the risks.

3. I hereby voluntarily release, forever discharge, and agree to indemnify and hold harmless Melt Mobile Infrared Sauna from any and all claims, demands, or causes of action, which are in any way connected with my participation in Melt Mobile Infrared Sauna activities or my use of Melt Mobile Infrared Sauna equipment or facilities including any such claims which allege negligent acts or omissions of Melt Mobile Infrared Sauna.

4. Should Melt Mobile Infrared Sauna or anyone acting on its behalf, be required to incur attorney's fees and costs to enforce this agreement, I agree to indemnify and hold it harmless for all such fees and expenses if Melt Mobile Infrared Sauna prevails in its defense of this agreement.

5. I certify that I have adequate insurance to cover any injury or damage I may cause or suffer while participating. Otherwise, I agree to bear the costs of such injury or damage myself. I further certify that I am willing to assume the risk of any medical or physical condition I may have.

6. I agree that if any portion of this agreement is found to be void or unenforceable, the remaining portions shall remain in full force and affect.By signing this document, I acknowledge that if I am injured during my participation in this activity, I may be found by a court of law to have waived my right to maintain a lawsuit against Melt Mobile Infrared Sauna on the basis of any claim from which I have released it herein. I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms.

INFRARED SAUNA AGREEMENT/ACKNOWLEDGEMENT

The use of drugs, medication or alcohol prior to or during the full spectrum infrared sauna session may lead to dizziness or unconsciousness. Clients using any medications must consult a physician or pharmacist prior to the use of the sauna.

Please consult your physician if you are in doubt of your ability to use the full spectrum infrared sauna for health reasons.

No one under the age of 18 is permitted in the full spectrum infrared sauna unless accompanied by a supervising adult.

Discontinue the use of the sauna if you feel light-headed, dizzy, or heat exhausted.

Sauna sessions should be limited to no more than 45 minutes and temperatures must stay below
165 degrees Fahrenheit.

Plastic water bottles are not permitted in the sauna.

Pregnant women should consult their physician prior to the use of the sauna. Excessive body
temperatures have a potential for causing fetal damage during the early days of pregnancy.

Medical Conditions/ Medications
IF ANY OF THE BELOW APPLY TO YOU, CONSULT YOUR PHYSICIAN PRIOR TO INFRARED SAUNA USE:

Have a fever

Recently had surgery

Any use of drugs, alcohol, or medication

Predisposed to bleeding

Have any flesh wounds

Have or have had cardiovascular issues

Insensitivity to heat

Have a pacemaker and/or defibrillator

Individuals who are using prescription drugs should seek the advice of their physician or a pharmacist for possible changes in the effect of the drug when the body is exposed to infrared waves or elevated body temperature. Diuretics, barbiturates, and beta-blockers may impair the body's natural heat loss mechanisms. Anticholinergics such as amitriptyline may inhibit sweating and can predispose individuals to heat rash or, to a lesser extent, heatstroke. Some over-the-counter drugs, such as antihistamines, may also cause the body to be more prone to heatstroke.

Proper Hydration and Consumption of Food
In all situations, it is required that you be hydrated for your sauna session. You must have appropriately nourished your body before your sauna session as well. Failure to do so could result in lightheadedness or unconsciousness.

Implants
Metal pins, rods, artificial joints, or other surgical implants generally reflect infrared waves and thus are not heated by this system. Nevertheless, you should consult your physician before using an infrared sauna.

PaceMaker / Defibrillator
Consult with your physician before using an infrared sauna. In the rare event that you experience pain or discomfort, IMMEDIATELY discontinue sauna use.

Agreement to Follow Directions
I agree to observe and obey all posted rules and warnings and further agree to follow any oral instructions or directions given by a "Melt Mobile Sauna" representative. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest Melt Mobile Sauna rep immediately.
Assumption of the Risks
I recognize on behalf of self, spouse, heirs, estate and assigns that the risk of injury from the activities involved in sauna use or cold plunge is significant. I recognize that severe injuries, including permanent paralysis or death can occur in activities including but not limited to breath work, training routines with ice/cold/heat, extreme cold, ice baths, condition of participants, equipment, or other factors. Potential injuries include but are not limited to heart failure, loss of consciousness, and stroke and may be caused by extreme cold, ice baths, weather conditions, condition of participants, equipment, or other factors.
